IN-HOME SERVICES
Our in-home therapists and behavioral assistants work collaboratively with the Child & Family Team, designing a plan to help better manage intense and challenging behaviors in the home and at school.  Services can be initiated through PerformCare (877) 652-7624. We also offer Bio Psychosocial Assessments.
ACCESSING SERVICES
There are a few different ways to begin services.
Tara Prezioso LCSW LLC is a medicaid provider through the NJ Children's System of Care (CSOC). That means you can begin by contacting PerformCare at (877) 652-7624 and requesting a BioPsychoSocial evaluation be completed by Tara Prezioso LCSW (provider #511854). If the evaluation finds that in-home services are appropriate, we can then provide the in-home therapy.
Reach out to us directly for a phone-screen to discuss challenges and whether or not therapy makes sense. We offer a sliding scale, fee-for-service or, if you have out-of-network benefits, you can submit to your insurance company.
